With an approved referral from an in-network Provider, benefits can be used the same at a Specialist as at a network office.
HIGH ANNUAL MAXIMUM
$2,500 of annual benefit per individual
Use towards general in-network treatment, referred specialty treatment, & commonly excluded services like dentures & implants
EMERGENCY
BENEFIT
When more than 100 miles away from the closest network office and have a dental emergency, your Emergency Benefit (up to $100) can be used to relieve any pain.

Call the office or submit an Appointment Request Form online. During this step, be sure to let the team know you are a Northwest Dental Benefits Member.

The team can access your plan information using your name, birthday, and employer.
No need to wait!
Your account information is stored in-house, so you will not receive a physical membership card. No need to wait to schedule an appointment- get started right away!
Frequently Asked Questions
FREQUENTLY ASKED QUESTIONS
Q: When will I receive my Membership Card?
Since all in-network offices can access your information using your name, birthday, and employer, we do not issue physical Membership Cards. Our team members will verify your eligibility and benefits ahead of time or at the time of your appointment.
Q: How do I use my orthodontia benefit?
Northwest Dental Benefits Members are welcome to start orthodontia anytime and do not require a practice referral.
Treatment from In-Network Orthodontists is eligible for up to $2,500 in benefit. If NWDB does not have a local option within a 5 mile radius, Members can elect the Orthodontist of their choice for a benefit of up to $2,000.
There is a six (6) month disbursement waiting period, meaning benefits will become eligible for payout 6 months after the initial enrollment date.
(Ex. If you joined Northwest Dental Benefits in January, your benefit will become eligible for payout in July.)
Please note, this does not mean you cannot start right away. You are welcome to begin treatment anytime!
Q: When do my benefits reset?
With a few exceptions, most plans' Annual Maximums reset on January 1st of each year. Contact your HR Department for your group's specific plan year. Your Lifetime Orthodontia Maximum does not reset.
Q: Where can I find a complete list of covered services?
A copy of your Detailed Plan Summary, which includes a list of covered services and associated copays, is available through your HR administrator.
